Cardiometabolic Health After 40: What Weight Loss Leaves Out

Your trousers fit again, and the bathroom scale finally seems cooperative. That is satisfying, especially if losing weight has taken months of effort. But a smaller number cannot tell you whether your liver, blood pressure and cholesterol have all improved together.

For cardiometabolic health after 40, the useful question is broader: what changed inside your body, and how would you know?

Emerging evidence: equal weight loss can produce different liver benefits

A recent diet trial offers a particularly clear reason to ask.

Researchers at Washington University School of Medicine assigned 55 adults with obesity, prediabetes and excess liver fat to ketogenic, Mediterranean or very-low-fat plant-forward diets. Food was provided, with weekly dietitian support. Across roughly five months, the groups lost approximately 10% of their starting weight. Yet liver fat fell by about 67% with the ketogenic diet, compared with 45% on each alternative. These were relative reductions from baseline, not percentage-point changes. WashU Medicine, 2026.

The study also found similar improvements in muscle insulin sensitivity across diets, alongside greater improvements in liver insulin sensitivity with carbohydrate restriction. Its published abstract reported no significant differences between groups in LDL cholesterol or apolipoprotein B. Petersen and colleagues, *Cell Metabolism*, 202600293-7).

This is emerging evidence for a specific dietary advantage in a specific population. It does not establish that ketogenic eating extends life, prevents heart attacks or suits every adult over 40. Nor does a small trial finding no significant lipid difference establish long-term cardiovascular equivalence.

The distinction matters. A diet can produce a meaningful liver benefit without answering every question about cardiovascular health.

A second trial reveals why checking the whole picture matters

Stanford's Keto-Med crossover trial provides a useful counterweight. Adults with prediabetes or type 2 diabetes followed ketogenic and Mediterranean-plus diets for 12 weeks each.

Among 33 participants with complete data, HbA1c did not differ significantly between diets. Triglycerides fell more during ketogenic eating, but LDL cholesterol rose approximately 10% from baseline, compared with a 5% reduction during Mediterranean-plus eating. These trials used different designs and populations. They should not be treated as a direct contest. Together, they support a practical interpretation: an improvement in one metabolic measure does not guarantee improvement in another.

Established evidence: blood pressure and LDL deserve independent attention

Some evidence reaches beyond laboratory changes to clinical events.

In the NIH-funded SPRINT trial, 9,361 adults at increased cardiovascular risk, without diabetes, were assigned different systolic blood-pressure targets. Participants were at least 50 years old. Intensive treatment reduced the annual rate of the main cardiovascular outcome from 2.19% to 1.65%.

That benefit came with more serious episodes of low blood pressure, fainting, electrolyte abnormalities and acute kidney injury. SPRINT supports treating blood pressure seriously; it does not provide a universal target for unsupervised use. SPRINT Research Group, 2015.

LDL lowering also has evidence from clinical outcomes. In FOURIER, 27,564 patients with established cardiovascular disease received evolocumab or placebo alongside statin therapy. Over a median 2.2 years, the primary cardiovascular endpoint occurred in 9.8% versus 11.3%, respectively. Sabatine and colleagues, FOURIER investigators, 2017.

Those participants already had cardiovascular disease. Their absolute benefit cannot simply be transferred to a healthy 45-year-old.

Still, the broader lesson is supported by established evidence: blood pressure and LDL are worth assessing in their own right. A favourable weight trend cannot substitute for either measurement.

Build a record that can answer a useful question

You do not need to turn your kitchen into a research unit. You need comparable observations and a clear reason for collecting them.

1. Start with a baseline you already have

Gather your recent blood-pressure readings, lipid results and any glucose or HbA1c measurements already obtained through routine care. Record dates and units.

Add the dates of major changes in eating, activity or prescribed treatment. Without that context, two laboratory reports several months apart can tell an incomplete story.

Discuss which missing measurements would actually inform a decision. This is preparation for a clinical conversation, not a universal shopping list of tests.

2. Make home blood-pressure readings comparable

The American Heart Association's measurement statement recommends a validated upper-arm device with an appropriately sized cuff. Empty your bladder, rest quietly for five minutes, support your back and arm, and keep your feet flat. Avoid caffeine, exercise and smoking for at least 30 minutes beforehand.

A commonly recommended home schedule uses two readings at least one minute apart, morning and evening, over seven days. Keep every reading and review the average with your clinician. American Heart Association, 2019.

The purpose is a trustworthy record. Repeating measurements until a reassuring number appears defeats it.

3. Define success before changing your routine

Write down what you want to learn. “Can I maintain this eating pattern?” is a different question from “Has my LDL changed?”

Keep the observations separate:

  • Behaviour: meals prepared or planned walks completed.
  • Daily function: hunger, sleep quality and ability to complete your usual activities.
  • Clinical measurements: blood-pressure averages and relevant laboratory results, repeated at an interval agreed with your clinician.

A routine may be easy to sustain without improving every marker. A marker may improve while the routine proves impossible to maintain. Both findings matter.

Personal experimentation: test feasibility, not your lifespan

Personal experimentation belongs in a narrower category than established or emerging clinical evidence.

Suppose Elena, a fictional 52-year-old, has changed her eating pattern and lost weight. She feels pleased, but her next results show lower triglycerides alongside higher LDL.

Her weight loss remains real progress. The LDL result remains worth discussing. Neither observation cancels the other.

For her next personal experiment, she might test whether preparing weekday lunches makes her agreed eating plan easier to follow. She could count lunches prepared and note afternoon hunger over a fortnight. Those are observable results within her control.

She cannot determine from that fortnight whether she has prevented a future heart attack. Nor could she confidently credit lunch preparation for a laboratory change if her medication and exercise changed simultaneously.

Personal records help you notice patterns and ask better questions. They do not turn one person into a randomized trial.

Mistakes to avoid

The scale-only verdict. Calling a programme successful solely because weight fell overlooks the separate questions raised by blood pressure, lipids and glucose regulation.

The favourite-marker defence. Pointing to improved triglycerides whenever LDL rises selectively edits the evidence. Bring both results to the same discussion.

The population leap. Applying a trial in adults with obesity, prediabetes and fatty liver to a lean, metabolically healthy adult assumes an answer the trial did not provide.

The multiple-change blur. Starting a diet, supplements and a new training programme together makes attribution difficult. If changes are clinically necessary, record them; do not delay treatment to preserve an experiment.

The measurement chase. Keeping the lowest blood-pressure reading or comparing measurements taken under different conditions creates apparent progress that may be procedural.

Before your next health review

Choose one practical task this week: assemble your existing results, or prepare a properly collected home blood-pressure record if relevant to your care.

Bring a specific question: “My weight has changed. Which other results improved, which did not, and what deserves follow-up?”

That conversation gives your effort a more useful destination than the next number on the scale.
